Dementia or Memory Impaired
With our Certified Dementia Practitioner background, we are able to tailor the plan of care according to the extent of the client’s memory impairment. We plan the care with regards to engagement activities including brain exercises.
Acute Myocardial Infarction
Plan of care for AMI clients will include a cardiac rehabilitation. This will include regular exercise program as tolerated by the client and proper management of stresses will implement energy conservation techniques that prevents too much work for the heart.
Congestive Heart Failure
Our caregivers will work with the family, client and home health agency in monitoring the client’s food, fluid restrictions, and weight gain monitoring. Getting the patient’s weight in the morning in clients with CHF will dramatically decrease hospitalization due to fluid retention.
High Risk for Falls &
Fall Prevention
Our fall prevention program includes an in depth fall risk assessment starting from the environment and the client’s ability to perform ADLs safely. We modify the client’s environment to increase safety. We work with a DME company to help provide safety equipment for the client.
Coronary Artery Bypass Graft or Orthopedic Surgery
Our caregivers are trained in infection control after a surgery. Recovery from a surgery is vital when getting home from the facility. We will implement our exercise program or exercise program from the rehab facility will be continued at home for even faster recovery. This will greatly decrease the complications from the surgery.
COPD, Pneumonia, and Respiratory Illness
Implementation of respiratory exercises such as deep breathing exercises and pursed lip breathing can help manage COPD. History of COPD puts someone high risk for respiratory infection. Infection control by keeping the client’s environment clean and keeping hands clean has proven to decrease infection.
